Aurora's basic attacks give passive stacks but don't trigger it. I guess it's because the game thinks aurora still has 4 stacks even tho it was changed to 3.
Ways to reproduce the bug:
1) Find a mayhem game
2) Pick Aurora
3) Load in the match and find a target to shoot
4) Shoot the target
5) At 3 full stacks, Aurora just keeps casting basic attacks without triggering her passive.
Possible solutions:
1) If my guess about game thinking Aurora still has 4 stacks is correct, then change it to 3.
2) Give Aurora 4th stack back in mayhem mode.
